Online Forms

You can access electronic forms and complete them online using OneView:

Instructions for Creating an Account

  • Click on "Create a New Account" on the left
  • Fill in the Personal Information
    • It is recommended that you create a username and password you will remember to access forms in future years.
  • Link Students to your account.
    • You should have received an invitation code (for use in Option #1) or your student ID (for use in Option #2) from the school.  This was distributed during Meet the Teacher night.  If you did not pick it up, it will be mailed to you.  
    • Use this information to link the student with your account.
  • Once you have linked your accounts, click on "View Online Forms" at the top
  • Click on the "Student Demographic & EMA" form.
  • Fill out the form.  You can push the Save button to save changes and return later.  When you are complete, push "Save & Submit to District"

Common Questions

Question: My child is in Kindergarten through 2nd grade and will not receive a 1:1 Chromebook. They will only have access to the Chromebook cart. Do I need to purchase insurance from Worth avenue?
Answer: No. Only students receiving a 1:1 Chromebook are responsible for them (3rd grade through 8th grade). We recommend parents buy the insurance from Worth Avenue:
Question: My account is locked out because of failed attempts to login.  What can I do to reset it?
Answer: You can try shutting down your browser and restarting it.  If that does not work, you will have to wait 1 hour for it to get unlocked.


Question: I receive an error when I try to submit the form:
Part One: Student Email Address must be your email address. Check the email address provided. 
Answer: Several families encountered this early in the week (Monday Aug 28 to Aug 29).  This was fixed Tuesday August 29.  Please try again. 
Question:  I receive an error as soon as I click on the link for the form ("Student Demographics & EMA").  
 Server Error in '/' Application.
The given key was not present in the dictionary.

Question: Is there a way to edit the form after you have already submitted it?
Answer: This has been changed so it is now possible.  It was not earlier in the week (Aug 28-29).
Question: Why does it jump to an unrelated section of the form when I push TAB to go to the very next field.
Answer: This was fixed on Tuesday August 29, 2017
