When completing the application, you'll be prompted to include the following additional documentation:
Preschool & Pre-K
- For Preschool 3's and 4's, you will need to choose which session (AM or PM) to apply for. See our Preschool Page for schedule details.
- Current photo of your child
- Permanent Record Card - required by the Diocese of Cleveland. You can download this form, complete it electronically, and attach it to the application.
- $175 application fee
Kindergarten
- Current photo of your child
- Permanent Record Card - required by the Diocese of Cleveland. You can download this form, complete it electronically, and attach it to the application.
- Copy of your child's Birth Certificate
- Copy of your child's Baptismal Certificate (if applicable)
- ETR/IEP or 504/Service Plan (if applicable)
- Custody documents (if applicable)
- $200 application fee
Grades 1-8
- Current photo of your child
