Early Learning Center Enrollment

Enroll in Preschool or Pre-K

Listed below is the additional documentation required on the Preschool & Pre-K application:

  • For Preschool 3's and 4's, you will need to choose which session (AM or PM) to apply for. For reference, our Preschool schedule is as follows:
    • 3's AM: Tuesday/Thursday 8:00 a.m. - 10:30 a.m
    • 3's PM: Tuesday/Thursday 12:00 p.m. - 2:30 p.m.
    • 4's AM: Monday/Wednesday/Friday 8:00 a.m. - 10:30 a.m. 
    • 4's PM: Monday/Wednesday/Friday 12:00 p.m. - 2:30 p.m. 
  • Current photo of your child
  • Permanent Record Card - required by the Diocese of Cleveland. You can download this form, complete it electronically, and attach it to the application 
  • $175 application fee

Preschool & Pre-K Forms

The following form is required by the State of Ohio for all Preschool and Pre-K students and must be signed by a physician. All other forms will be completed electronically.

Child Medical Statement
